Directions for Filling out Interactive PDF Forms

Please read the instructions completely before filling out the form.

 

In order to access the form, you MUST have the Adobe Acrobat Reader software installed on your computer. The form is best viewed with the Adobe Reader version 5.0 or above. If you do not have the Reader, or if you are using version 4.0 or below, you can download the latest version of the Acrobat Reader free from the Adobe site.

Each form contains "fields" which allow you to type in data. The fields are located where you see large boxes or small checkboxes.

Use your mouse to click in a field and then type in your data.

IMPORTANT NOTE: Each form must be filled out completely while the file is open. The forms can be filled out while you are connected to the Internet, or you can save the file to the hard drive of your computer, then open it in your web browser window while working off line, fill in the fields and print. BUT you will not be able to save any data you type into the fields, either online, or on your hard drive. This means that you cannot fill in part of the form, then return later to complete it. Each time you close the document, any data you have added will be lost. If you save the form to your hard drive, anything you typed into the fields while online will not save onto your hard drive with the form. You must fill the form out in your web browser. You cannot open the file in a word processing program.

Some Things to Remember

  • You will be limited to the space that is visual in each field on the form. Anything typed outside the perimeter of the field will not print.

  • If you attempt to save the file by clicking on the "disk icon" in the Adobe menu bar or clicking on "File/Save" on your browser menu, you will save ONLY the form, not any data you have input.

  • Once you have filled in all inter-active fields, print the form and sign and date the form where applicable.
